Evonne L. Crump (DO – Maryland) is a certified family physician currently
serving as the Medical Director for the Concentra Medical Center in Lanham, Maryland.
A majority of the physicals performed in the center are CMV driver physical examinations.
Her duties include performance of pre-employment, return-to-work, and respirator
physicals and injury management. Her medical director responsibilities include staff
management, client support, and community education. She is dedicated to the education
of medical students and residents and currently serves as an adjunct professor of
family medicine for New York College of Osteopathic Medicine, as well as a resident
preceptor for the National Rehabilitation Hospital. Prior to joining Concentra,
Dr. Crump was the primary physician for an urgent care center where the majority
of patients were underserved. As a resident, Dr. Crump was essential in the formation
of programs for the medically underserved. She continues her service to the underserved
through a variety of community service volunteer efforts and education programs.
Dr. Crump currently conducts 2,500 CMV driver physical examinations annually.
Teri Gourley (DO – Ohio) is a board certified family physician currently
employed by Samaritan Regional Health System, in their hospital-based occupational
medicine clinic, in Ashland, Ohio. As part of her occupational medicine practice,
Dr. Gourley manages occupational injuries and conducts pre-employment physicals,
return-to-work physicals and respirator evaluations. She performs work site analysis
and helps coordinate and market health and wellness programs for the hospital’s
industrial clients. She lectures and gives presentations to the community and industrial
clients on health and wellness topics and industrial/occupational topics. She also
works in the hospital’s Emergency Department where she evaluates work injuries and
other minor emergencies. Dr. Gourley brings a background in managing acute and chronic
illness. Prior to occupational medicine practice, she was a National Health Corps
scholar and worked at a community health center. She has completed the American
College of Occupational & Environmental Medicine (ACOEM) Basic Curriculum in Occupational
Medicine series and participated in the American College of Physician Executives:
Physicians in Management coursework. Dr. Gourley conducts approximately 300 CMV
driver physical examinations annually.
Hetzal Hartley (MD – Virginia) is the medical director of the Carilion Occupational
Medicine Program, an occupational medicine clinic serving more than 400 client companies.
Dr. Hartley is also the Medical Director for Carilion Employee Health, an eight-hospital
health care system serving 10,000 full-time employees. He works on the Infection
Control, Employee Health, and Safety committees for this organization. He received
his masters in public health in occupational medicine in 1998, and is a certified
Senior Aviation Medical Examiner as well as a certified Medical Review Officer.
Dr. Hartley has taught for many years and currently serves in a formal capacity
as Clinical Assistant Professor of Emergency Medicine at the University of Virginia
School of Medicine as well as adjunct clinical faculty in Family Medicine Residency
at Roanoke Memorial Hospital. He has given presentations on topics such as workers
compensation, drug abuse, triage, trauma, toxicology, injury prevention, and industrial,
undersea, and aerospace medicine. He is retired from the United States Air Force
Reserve. He flew as a flight surgeon in both Iraq and Afghanistan, logging more
than 1,000 hours as a crew member during his military career. Dr. Hartley conducts
800 CMV driver physical examinations annually.
Karen Horn (APN – California) has specialized in occupational health since
1985, when she began work as a nurse practitioner at Hughes-Lewis Associates in
Oakland, California. For the past eight years, Ms. Horn has worked as a nurse practitioner
at Concentra Health Services. Prior to her work in occupational medicine, Ms. Horn
worked in gastrointestinal and infectious disease at Memorial Hospital-Sloan Kettering
Cancer Center in New York, NY. She has a master’s of science in nursing from the
University of California, San Francisco. Ms. Horn conducts 500 CMV driver physical
examinations annually.
Vaunzell Linnville (PA – Virginia) currently works at the Prince William
Hospital Emergency Department and Occupational Health Clinic. Ms. Linnville provides
urgent and emergent medical treatment for work-related illnesses and injuries, performs
patient physical exams including Department of Transportation (DOT), pre-placement,
and return-to-duty examinations, and performs evaluations and screenings for routine,
travel, and post-exposure vaccinations and immunizations. Prior to working at these
institutions, Ms. Linnville worked as a physician assistant in the U.S. Air Force
at Walson Air Force Family Practice Clinic in Ft. Dix, New Jersey, where in addition
to providing care to more than 44,000 beneficiaries, highlights of her work included
managing the Pseudofolliculitis Barbae (PFB) and Wart Clinics, serving as the medical
liaison for episode of care of depression, completing the Medicolegal Death Investigator
Training Course, and organizing the clinic’s first National PA Day. Ms. Linnville
also worked as a physician assistant at the family practice clinic on the Los Angeles
Air Force Base, where she served as Assistant Director of the Women’s Health Program,
as Supervisor of Minor Surgery Tracking, and as a member of the Infection Control/Patient
Assessment Team. Ms. Linnville has her master’s degree in physician assistant studies
in family practice and is certified with special recognition by the National Commission
on Certification of Physician Assistants.
Douglas Martin (MD – Iowa) currently serves as Medical Director for St. Luke’s
Occupational Health Services in Sioux City, Iowa, a hospital-affiliated occupational
medicine provider. He has worked in occupational health for his entire career, serving
as staff physician or medical director in each prior setting. Dr. Martin has served
in many positions for the American Academy of Disability Evaluation Physicians,
including President, Vice President, Treasurer, and Secretary. He currently serves
on the organization’s Board of Directors. In addition, Dr. Martin is the current
Medical Review Officer Section Chair for ACOEM. He teaches occupational medicine
and medical review officer courses for ACOEM and the American Academy of Disability
Evaluating Physicians (AADEP) and is a guest author for ACOEM’s MRO Update Newsletter.
He is board certified in family medicine and as an independent medical examiner
and serves as both a Medical Review Officer and FAA Medical Examiner. Dr. Martin
gained experience in the development and psychometrics of physician certification
examinations while serving on the exam committee for AADEP’s certification examination.
He conducts 150 CMV driver physical examinations annually.
Wendy Paracka (APN – Florida) is the founder and owner of Remise Health Solutions
in Tarpon Springs, Florida, an urgent care facility that provides occupational health
services, including industrial employment physicals, Occupational Safety and Health
Administration surveillance exams and DOT CMV driver physical examinations, as well
as care for acute and chronic patient conditions. Ms. Paracka has worked in a variety
of clinical settings, including a hospital, a medical center, and as the director
of a cardiac catheterization laboratory. She brings a background in urgent and primary
care, internal medicine, rheumatology, and sports medicine. Ms. Paracka is a member
of the American Academy of Nurse Practitioners, the American College of Nurse Practitioners,
the Florida Nurses Association, and the Florida Nurse Practitioner Network. She
conducts 125 CMV driver physical examinations annually.
Michael Tso (DC – Missouri) serves as President of Dr. Michael Tso, Inc.,
providing consulting services and conducting CMV driver physical examinations, breath
alcohol and substance abuse testing, physical demand assessment, and workman’s compensation
examinations. He is a founding member and current Vice President of HealthBridge,
LLC, an organization that provides evaluation and determination of permanent impairment
ratings, as well as the determination of medical causation. He is also an administrator
for CMV examinations for Road Ready, Inc. Dr. Tso has served as a seminar instructor
in emergency care procedures. He currently conducts 3,800 CMV driver physical examinations
annually.
Karl Wagner, Jr. (PA – Michigan) is the senior physician assistant at Providence
Hospital and Medical Centers’ Internal Medicine Department. In addition, he currently
provides physician assistant support at Corporate Health Services in Southfield,
Michigan. Mr. Wagner has worked in occupational health, substance abuse, home medical
service, emergency room, and industry medical environments. He has served on the
adjunct faculty at the University of Detroit-Mercy in the Physician Assistant Program,
as proctor for the National Commission on Certification of Physician Assistants,
Inc., and on the adjunct faculty for Wayne State’s Medical Dietetics Program and
Physician Assistant Program. Mr. Wagner has served as President-elect and President
of the American Academy of Physician Assistants in Occupational Medicine and as
a member of the Michigan Council on High Blood Pressure. In addition, he has filled
several positions with the Michigan Academy of Physician Assistants, including Fellow,
Legislative Committee Member, Board of Directors Region Six Representative, and
Chairman of the Membership Committee. Mr. Wagner has several years of United States
Coast Guard experience, including work as a hospital corpsman and military representative,
for which he received a National Defense Medal. Mr. Wagner conducts 300 CMV driver
physical examinations annually.
Jeffrey Wingham (DC – Illinois) received his doctorate of chiropractic from
Logan College in 1989. He has owned his own practice in Springfield, Illinois since
2000. At this multi-specialty pain center, Dr. Wingham employs a physical therapist,
a surgeon, and a medical doctor who is board certified in neurology, citing a belief
that more comprehensive choices for care better serve his patients. Dr. Wingham
has been DOT-certified for drug and alcohol testing since 2005. He is a certified
Breath Alcohol Technician. He has completed numerous post graduate studies on the
efficacy and hands-on application of Federal regulations and guidelines for performing
CMV driver physical examinations. He currently conducts 500 CMV driver physical
examinations annually.
